Truck DRIVER REQUIREMENTS
- Minimum age: 22
- 12 months of verifiable all weather experience (in last 5 years) driving a tractor/trailer or other vehicle required for the job you are applying for.
- Passed DOT physical in the last 12 months
- No more than 4 moving violations in the last 3 years. No more than one can be considered a serious violation under US DOT Standard
- No reckless/careless driving convictions in the last 3 years.
- No CDL driving suspensions in the last 3 years. Exceptions may apply.
- No Major Preventable Highway Accident in the last 5 years.
- No more than 2 minor preventable accidents in the last 3 years.
- No alcohol or drug related DUI/DWI in the last 5 years.
- No convictions for the possession and/or use of controlled substances or illegal drugs in the last 5 years.
- No convictions for the manufacturing and/or distribution of controlled substances or illegal drugs
- No felony, criminal or traffic convictions in the last 10 years. Exceptions may be reviewed.
- No positive DOT drug/alcohol tests in the last 5 years
EQUIPMENT & BUSINESS REQUIREMENTS
- Proof of ownership: Front & back of Title or Title application less than 45 days old in your name/company name.Lease Purchase: Front & back of the Title and a copy of the lease agreement.
- Proof of Non Trucking Liability Insurance. B&W Cartage Company, Inc. MUST be the Certificate holder. The address should be 9850 Pelham Road, Taylor, MI 48180. The certificate must show: Owner, Year, Make and Serial Number. Coverage must not be less than one million dollars ($1,000,000.00).
- Photocopy of apportioned cab card (Registration)
- Leasing a Base Plate
- Contractor Promissory Note and Plate Deduction Agreement.
- If applicable, current Schedule 1 (Form 2290) – Heavy Highway Vehicle Use Tax.
- A DOT annual vehicle inspection less than 45 days old.
- DBA, LLC or Incorporation papers.
- Satellite agreement
- Charge card agreement (if applicable).
- Broker must have a Federal Identification number.
- W-9 form
- Lease and equipment agreement.
- Permit Notice
- Signed Operating Agreement
